I, the undersigned, am aware that there are significant risks involved in all aspects of physical training. These risks include; falls which can result in serious injury or death; injury or death due to negligence on the part of myself, my training partner, or other people around me; injury or death due to improper use or failure of equipment; strains and sprains. I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or death that may result from participation in any activity or class while at, or under direction of Vinse Active and its partners, I acknowledge that I have no physical impairments, injuries, or illnesses that will endanger me or others.
I recognise that there is risk involved in the types of activities offered by Vinse Active and its partners. Therefore I accept financial responsibility for any injury that I may cause either to myself or to any other participant due to my negligence. I further agree to indemnify and hold harmless Vinse Active, their principals, agents and employees from liability for the injury or death of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in activities offered by Vinse Active and its partners. I have read and understood the foregoing assumption of risk, and release of liability and I understand that by signing it obligates me to indemnify the parties named for any liability for injury or death of any person and damage to property caused by my negligent or intentional act or omission.
Information for parents and carers
Vinse Active aims to provide a safe and enjoyable experience for every child or young person. To help us do this, please note the following important information:
We recognise the need to ensure the welfare and safety of all children. We have written a policy statement which sets out how photographs and videos of children involved in our activities should be taken and shared. A copy of this is available from [VinseActive.com].
We will take all steps to ensure images of children are used solely for the purposes they are intended. If you become aware that these images are being used inappropriately you should inform a members of Vinse Active immediately. We will not take or share photographs, video or other images of children and young people without the consent of the children involved and their parents or carers. This agreement fits with our overarching online safety policy. If you would like to know more about this, please speak to a member of our staff.

I confirm that I have read Vinse Active’s photography and filming policy and I agree to my child’s photograph being used within the Vinse Active’s platforms for display purposes my child’s photograph being used within other printed publications my child’s photograph being used on the Vinse Active’s website my child being videoed for use onVinse Active’s website my child’s photograph being used onVinse Active’s social media pages my child being videoed for use on Vinse Active’s social media pages.
I understand the potential risks associated with the use and distribution of these images how these images or videos will be stored within the organisation and how long for that if I withdraw consent for my child’s image to be used or shared in the future, it may not be possible to remove images that have already been published or distributed that at many events, Vinse Active and others will reasonably wish to take wide angle
general photos during or at specific points in the event that I must gain permission before sharing photographs/videos of other people’s children on social media or that I must not share photographs/videos taken during Vinse Active’s events on social media that if I share images of my child with friends and family, I should ask permission from their parents, Vinse Active staff and check the privacy settings of my social media account first to understand who else will be able to view.
